Who is Dr. Diaz?

Dr. Richard P. Diaz has been a Family and Psychiatric Mental Health Nurse Practitioner for the last 23 years. He received his Doctoral Degree in Nursing Practice from the University of Tennessee, Health Science Center, Memphis in 2009. Dr. Diaz spent 20 years in the Military where he retired as a Major in the United States Army Nurse Corp. Dr. Diaz had the privilege of working with the Wounded Warrior’s at Brooke Army Medical Center Delta Company, Burns and Amputee’s Company. Dr. Diaz has a strong background in Family Practice and Wellness. Dr. Diaz has been an online and face to face nursing profession for the last 10 years. 

How does it work?

Dr. Diaz is currently holding all appointments through Zoom.

Dr. Diaz is available weekly on Tuesdays from 5pm to 7pm. 

The services offered are similar to those offered in a CVS minute clinic. Any emergency issues should be handled via the ER. 

This is a free service for the most part for our employees. Our company will pay the fee for Dr. Diaz. If specific tests are needed or any bloodwork, there may be a fee. Please always ask for clarification. 

Appointments can be made through Crystal Ogburn. You will not have to disclose the nature of wanting to see Dr. Diaz as your reason for the visit is confidentialOnce Crystal receives your email, you will get an invite from outlook with the time, Zoom meeting ID and zoom passwordPlease remember if you need to cancel to let Crystal know as soon as possible, so she can book someone else in your place.
